1 On every visit you must show:
• Personal identification for all dependents living in your home; such as * valid health card (not photocopies), drivers licence (photocopies for dependents allowed); cheque stubs (ODSP/OW) showing dependent's names; school report cards with current address. Blue hospital cards, birth certificates & health cards presented on cell phones are not acceptable.
• Food is provided for the number of people you can prove live in your household.
• Proof of income including child tax credit & housing costs will be required once a year.
*By law, you are not required to produce your health card nor can the health card number be recorded but if the card is offered voluntarily it can be accepted as identification.
2 Once per month, St.Joseph's Food Bank will supply you with about 3 days worth of groceries. We serve the residents of Pembroke and Laurentian Valley. If you live outside of this area, please see the information about other food banks on the reverse side of this sheet.
3 Lunches are available for full time students and part-time dependents with no i.d. (visiting weekends, 50% custody, etc). Lunches may contain nut products.
4 Please bring your own reusable, clean grocery bags and check all food orders for any missing items before leaving the hall.
5 If you are unable to come to the food bank, you may call 613 732-3807 to advise us of the person picking up food on your behalf. A signed note and applicable identification must also be sent.
**Abusive language and behaviour will not be tolerated**.
